My 1999 Nissan Pathfinder (3.3L standard) has a P0158 DTC, indicating that max voltage has been exceeded in, I believe, the rear, driver's side O2 sensor. The STFT values for banks 1 and 2 are 0% and -100.78% for banks 3 and 4. The LTFT values are -7.03% for banks 1 and 2 and -100.78% for banks 3 and 4.

First, do banks 3 and 4 related to the rear post-converter sensors? If that is the case, it looks like I may actually have a problem with both. Second, is it likely the O2 sensor(s) is/are shot, or is this an indication that something else might be wrong. The car has hit 250,000 km, so I think it's time to replace them anyway, but I want to know what will get me through aircare. Thanks.
