Clutch Noise

It seems my clutch makes a sort of buzzing sound when I depress the pedal at high rpm. It only makes the sound when I'm in the process of pressing (as I'm moving it) and at 5000+ rpm. Also, you can feel very minor vibrations. What is this? Any ideas?

It's been this way for a few months now.

Any help would... help =)
