So my 2004 4WD Titan goes through all gears (and operates fine) in forward. In reverse, it will only move if on an incline with gravitational assistance. It only has 128K miles. Is this a common Titan transmission issue? Thoughts on whether tranny needs repair or replacement?

There are several tests that would have to be made in order to properly isolate your problem. In the factory service manual, there is a symptom chart in the A/T section and the possible culprits would be listed under the "No Reverse" symptom. A couple of things that are known problems include snap-rings breaking in the reverse clutch pack and bad reverse solenoids in the valve body OR broken pins on the TCM, which mounts to the valve body. The later two would have stored trouble codes in the TCM.
