Sounds like the cam chain tensioner is going out. Do a search for cam chain tensioner and you'll find more info about it. Could be the top chain guide has broken, and if so you can remove the valve cover and remove it as it's really not needed. I think Nissan had a TSB that said to remove it anyway.
