Dan,
Transmission flushes are important on all automatic transmissions. If there hasn't been much in the way of transmission maintenence (pan drops, filter changes, fluid change) every two years or 30K, 120K is probably about average for a failure.
I'd avoid aftermarket transmission rebuilds! They typically don't last as long - Nissan has a facility that does nothing but reman transmissions on an assembly line. An independent just can't compete with that.
You might try a flush, but usually it's too late when something fails.
I'd try to purchase a new or remanufactured transmission from Joe at Infiniti of Scottsdale (1-888-216-5328) and then have it installed locally at a reputable shop. Joe sells wholsale to the public, and is a first class guy to work with! You'll typically get 25-30% off of dealer prices.
Since it's just a bolt on, bolt off situation, any competent shop should be able to handle the swap. You may also want to install an aftermarket aux. cooler if the only one on the truck is the one in the radiator. Heat is the big killer of transmissions, and the factory cooler actually acts as a heater when it's really hot. Using Mobil 1 ATF should allow you to bypass the factory cooler (heater) even in the winter since the viscosity is stable over a wide range. In extremely cold climates it may make sense to run the two in series.
Good luck!
Heath
