Well, you can always become part of a poll but I am not sure how useful you will find this information for your application. Since you obtained this tranny from a friend, I would suggest one of two thing.
#1 replace what I have already suggested(5th gear selector fork and gearset) Trying to do a "patch" job will bring you a head ache later. It is better to just fix the problem at it's source the first time. It makes no sense to do a patch job on it. Trust me, I have been around the business for many years. Since the trans is apart, you may as well replace bushings, seals and start from scratch. Don't repair, just rebuild.
OR
Take the tranny to a qualified professional. AAMCO can offer you a lifetime warranty on any transmission that they build. Yes this will be more expensive but as long as you own your car, they will become responsible for making sure it functions properly. Typically this is suggested for auto trans owners due to the internals involved but this is not limited by any means to your application.
I am not certain of your abilities so I cannot suggest anything else. If it were me, I would rebuild the tranny myself.
