Help! Driver door sagging badly!

I have changed door hinges in older American Cars using a jack, and it was pretty easy once the door was balanced. I notice there is a sort of lever/hinge in between the 2 big hinges. Do I have to remove this, and how?
Is there anything else to look out for? Also, the dealer prices are rediculous, where would I find a good source for replacements. 3rd, from slamming the door shut once in the car, the inside door handle is cracked in 2 places. Where would I find an inexpensive replacement? Thanks!

I can't tell you off the top of my head, but there is a writeup on www.se-r.net describing the hinge replacement.

Thanks, Adam, that's a really good site. Seemed like the consensus was to pay the dealer $100 to do it, or use the jack method, unless you have all kinds of autobody tools to align the door with.
