i have this problem too on a 94
but i know i have an engine probleme with the timing chain which i have for over a year now but tends to happen more often now the probleme began the same way your trucks are acting high idling anytime anywhere any weather i ran the truck for a year like this without taking much care there was a racling sound when starting the truck for about 2-3 sec i tough it was a normal problem due to the winter temperature making my valves knoking by now the sound doesnt stop it come from the front of the engine i got poor economique tank per miles i could get around 500km with a tank and now i get 400km so i am gonna replace the timeing chain causing this trouble (which is a well known problem on those ka24e engines) here the parts are sold around 140$CDN i got the kit for 60$CDN on ebay so its worth time to look for the parts
for your information the ratling sound is jsut louder when you start the truck so you can start it open the hood and listen if it can be your problem
